Extended Care Services:

San Bernardino Medical Group Extended Care provides convenient medical care for illnesses requiring immediate care, when your primary care physician is not available or when care is needed after regular business hours.  Our Extended Care is open Monday-Friday 9:00am-9:00pm, Saturday and Sunday 9:00am-5:00pm (excluding Thanksgiving and Christmas).

Health Education:

The Patient Relations department offers a variety of health education classes and programs to include Smoking Cessation, Diabetes Management and Coumadin Safety.  The education classes are offered to San Bernardino Medical Group patients, their families and friends.  Class flyers are distributed quarterly and monthly.

Choosing Your Doctor:

Your primary care physician will oversee all of your health care including preventative care, physical exams and specialty referrals.

You will need to call your health plan to select a San Bernardino Medical Group physician.  Usually you must request a physician transfer by the middle of the month for the change to be effective with your insurance by the first of the following month.

Family Practice and Internal Medicine physicians are available as primary care physicians.  San Bernardino Medical Group has a staff of 24 physicians to include 8 specialty physicians.

Insurances accepted by San Bernardino Medical Group

Once your insurance becomes effective with San Bernardino Medical Group, you will need to call and schedule an appointment to establish with your new primary care physician.  The appointment lines are:

  • Family Practice:  909-883-8611, Ext. 15030
  • Internal Medicine:  909-883-8611, Ext. 15005
  • Internal Medicine/Pulmonary:  909-883-8611, Ext. 15000

You may also call the Patient Relations Department for more information about physicians or to arrange a transfer from one physician to another within the group.

Medical Records:

If you are a new patient, you will need to complete a medical records release form to have your records transferred from your previous physician.  Please complete the form as soon as you join San Bernardino Medical Group so your physician has your complete medical history.  You may download your Medical Records Release form-see below, complete it and fax or mail it to your previous physician’s office.

Prescription Refills:

When you need a medication refilled, call your pharmacy to make the request.  Once you have contacted the pharmacy, they will contact the refill clerk at San Bernardino Medical Group.  The clerk will place your request into the system for your physician’s approval.  The pharmacy will then be contacted with the approval.  If your request is denied, your doctor or nurse will notify you.  Please allow 3-4 working days for the refill procedure.  Do not wait until you are completely out of medication to request a refill.  Some medication requests will require a prior authorization from your insurance company or may require a visit to your doctor’s office.  For mail order prescription programs, please follow the instructions provided by the insurance carrier.
