The Medical Provider Portal is used for providers with an established relationship with SBMG.  Once you have created an account with us you can view claims, referrals, and patient information on patients we have referred to you.  To setup an account please call the SBMG IT Liaison at (909) 883-8611, ext. 1-2338 during standard business hours.
